PENGERTIAN
Hidrocephalus merupakan suatu keadaan dimana terjadi penimbunan cairan cerebrospinal dalam pantrikel otak, sehingga kepala semakin membesar serta terjadi pelebaran sutura-sutura dan ubun-ubun. Cairan yang tertimbun dalam ventrikel biasanya antara 500-1500 ml, akan tetapi kadang-kadang dapat mencapai 5 liter. Hidrocephalus sering kali disertai dengan kelainan bawaan lain seperti spinabifida. 

Hidrosefalus merupakan suatu keadaan dimana terdapat timbunan liquor cerebrospinalis yang berlebihan dalam ventrikel-ventrikel,yang disertai dengan kenaikan TIK,sehingga terlihat kepala bayi busung atau membesar (Suryanah,1996:135)


KLASIFIKASI
Menurut waktu pembentukan :

  • Hidrosefalus yang terjadi pada neonatus atau yang berkembang selama intrauteri(kongenital) 
  • Hidrosefalus akuisita terjadi setelah masa neonatus atau disebabakan oleh faktor–faktor lain pada masa neonatus

Menurut sirkulasi CSS :
  • Hidrosefalus Komunikans
Hidrosefalus yang memperlihatkan adanya hubungan antara CSS sistem ventrikel dan CSS dari ruang subarakhnoidalis terhambat.Gangguan absorbsi CSS dapat disebabkan sumbatan sisterna subaroknoid disekeliling batang otak atau obliterasi ruang subarakhnoid sepanjang otak, seluruh sistem ventrikel terdistensi
  • Hidrosefalus Non komunikan / Obstruktif
CSS sistem ventrikel tidak berhubungan dengan CSS ruang subarakhnoid misal aquaduktus sylvii menyempit atau tersumbat.Terdapat hambatan sirkulasi CSS dalam sistem ventrikel sendiri akibatnya cairan ventrikal tidak dapat mencapai ruang subarakhnoid.Terjadi pembesaran sistem ventrikel di proksimal obstruksi.

ETIOLOGI DAN PATOFISIOLOGI
Penyebab hidrocephalus terjadi bila terdapat penyumbatan aliran cairan cerebro spinal pada salah satu satu tempat antara tempat pembentukan cairan cerebro spinal dalam sistem ventrikel dan tempat absorbsi dalam ruang subarackhnoid. Akibat penyumbatan, terjadi dilatasi ruangan cairan cerebro spinal diatasnya. Hidrocephalus disebabkan oleh 3 faktor 1. Produksi cairan cerebro spinal yang berlebihan 2. Obstruksi jalur cairan cerebro spinal dan 3. Gangguan absorpsi cairan cerebro spinal.
Banyak factor penyebab trjadinya  hidrosefalus, termasuk tumor, malformasi vaskuler, abses, kista intraventrikular, pendarahan intraventrikuler, meningitis, stenosis aquaduktus, dan trauma serebri.


JUMLAH CAIRAN CEREBROSPINALIS
Dalam otak manusia orang dewasa jumlah cairan normal cairan cerebro spinal = 90-150 ml, anak umur 8-10 th = 100-140 ml, bayi = 40-60 ml, neonatus = 20-30 ml, prematur kecil = 10-20 ml



MANIFESTASI  KLINIS 
Manifestasi klinis dipengaruhi umur,penyebab dan lokasi obstruksi,gejala-gejala yang menonjol merupakan refleksi hipertensi intra kranial,rincian gambaran umum sebagai berikut : 
  1.  Perubahan tanda-tanda vital (penurunan denyut apeks, penurunan frekuensi pernafasan, peningkatan tekanan darah) 
  2. Muntah. 
  3. Peningkatan lingkar kepala 
  4. Iritabilitas
  5.  Letargi. 
  6. Perubahan suara tangisan (bernada tinggi) 
  7. Aktivitas kejang. 

Pada bayi :
1)      Pembesaran kepala secara progresif (diatas persentil ke-95).
2)      Bagian frontal tengkorak menonjol.
3)      Fontanela tegang dan menonjol (khususnya yang tidak berdenyut)
4)      Distensi vena superficial kulit kepala.
5)      Transiluminasi melalui tengkorak meningkat secara simetris.
6)      Mata turun kebawah (“sunset eyes”)

Pada Anak Lebih Besar
1)      Sakit kepala di dahi, mual dan muntah.
2)      Anoreksia.
3)      Atareksia.
4)      Kekakuan ekstremitas bawah.
5)      Kemerosotan prestasi sekolah atau kemampuan kognitif anak.
      Tanda dan gejala yang terjadi disebabkan oleh peningkatan intrakranial (TIK) dan bervariasi berdasarkan usia anak dan kemampuan tengkorak untuk mengembang.


PEMERIKSAAN DIAGNOSTIK

  1. Lingkar kepala 
  2. CT Scan : merupakan cara yang aman dan dapat diandalkan untuk membedakan hidrosefalus dari penyakit lain,dan untuk identifikasi tempat obstruksi aliran CSS. 
  3. MRI (Magnetic Resonance Imaging) :menunjukkan pembesaran ventrikel. 
  4.  Lumbal pungsi :Indikasi pada pasien dengan peningkatan TIK dan Mengukur tekanan cairan cerebrospinal


DIAGNOSA BANDING
  1.  Megalensefali 
  2. Efusi subdural kronik 
  3. Tumor otak 
  4. Granuloma Intra kranial 
  5. Hematoma subdural 
 
 
KOMPLIKASI
  1. Peningkatan Intra kranial 
  2. Kerusakan otak 
  3. Infeksi : Septikemia, Endokarditis, Infeksi luka, Nefritis, Meningitis, Ventrikelitis dan abses otak 
  4. Shunt tidak berfungsi
  5. Kematian
 
 
PENATALAKSANAAN TERAPEUTIK 
Tujuan pengobatan adalah untuk mengurangi hidrosefalus, menangani komplikasi mengatasi efek hidrosefalus atau gangguan perkembangan.
 
Pencegahan 
Untuk mencegah timbulnya kelainan genetik perlu dilakukan penyuluhan genetik, penerangan, menghindari perkawinan dengan keluaraga dekat, proses persalinan diupayakan fisiologis untuk menghindari trauma kepala ,SC.Suatu saat dipilih daripada menanggung resiko cidera kepala bayi. 
 
Medikamentosa 
Hidrosefalus dengan progresivitas rendah dan tanpa obstruksi pada umumnya tidak memerlukan tindakan operasi,dapat diberikan Asetazolamid dosis 40-75 mg/kg BB/24 jam mengurangi sekitar sepertiga produksi CSS dan terkadang efektif pada hidrosefalus ringan yang berkembang lambat. Pada keadaan akut dapat diberikan manitol.Diuretika dapat diberikan meskipun hasilnya kurang memuaskan.Isosorbide atau Furosemide mengurangi produksi cairan CSS.
 
Pembedahan atau operasi 
Berupaya menghubungkan ventrikel otak dengan rongga peritoneal yang disebut Ventrikulo-Peritoneal.Peritoneal Shunt ditujukan untuk hidrosefalus non komunikan (shunting) memerlukan pemantauan yang berkesinambungan oleh dokter spesialis bedah saraf, bertujuan untuk mengalirkan cairan CSS yang berlebihan dari ventrikel ke ruang ekstrakranial, contoh rongga peritoneum.atrium kanan dan ruang pleura.
Dilakukan operasi Torkildsen yang dibuat pintas stenosis aquaduktus menggunakan tabung plastik yang menghubungkan 1 ventrikel lateralis dengan sisterna magna dan ruang subarachnoid medula spinalis. Pada operasi ini tidak berhasil pada bayi karena ruangan–ruangan ini belum berkembang dengan baik. 



PENYULIT
Tergantung pada causa dan progresivitas hidrosefalus, komplikasi tindakan pemirauan berupa :
 1)    Buntu input dan output disebabkan oleh :
-     Melekatnya kateter pada otak
-  Tertutup pleksus koroid/dinding ventrikel jaringan otak bekuan darah karena peningkatan protein CSS.Buntu output disebabkan oleh tertutupnya omentum mayor.
 2)     Sewaktu operasi dapat terjadi hematom subdural
 3)     Infeksi dapat terjadi 20-70 % dari kasus tergantung :
-     Pengalaman operasi
-     Jenis pemirauan
4)     Epilepsy terjadi sebagai akibat infeksi /tindakan operasi itu sendiri






AKIBAT HIDROCEPHALUS
Tanpa tindakan operasi, penimbunan cairan akan mengakibatkan penekanan pada jaringan otak normal dan selanjutnya akan mengganggu berbagai fungsi otak, termasuk fungsi-fungsi alat vital yang dapat mempengaruhi jantung dan paru.
